Primary Care Physician (MD/DO) - Summary- Outpatient primary care for seniors in a value-based care model
- Focus on complex patient population with multiple chronic conditions

- Provide comprehensive primary care services to a senior patient population with complex health needs
- Manage a panel of up to 650 patients, averaging 10-12 patients per day
- Utilize a value-based care approach to improve patient outcomes and satisfaction
- Collaborate with a dedicated care team, including medical assistants, schedulers, and referral specialists
- Participate in regular team meetings and quality improvement initiatives
- Coordinate with other healthcare professionals to ensure seamless care transitions

- Adhere to evidence-based clinical guidelines and organizational policies
