As a Primary Care Physician (MD/DO), you will play a vital role in ensuring that each patient receives comprehensive, personalized healthcare. You will have the opportunity to work closely with other providers, including Nurse Practitioners and a multidisciplinary care team, in a supportive setting that prioritizes preventive medicine, chronic disease management, and coordinated care. The practice is equipped with onsite laboratory services, access to specialty consults through RubiconMD, and electronic medical record systems such as Canopy and Greenway, streamlining workflow and enhancing the patient experience.

Primary Care Physician (MD/DO) - Summary
- Full-time, patient-focused role in primary care
- Provide equitable, value-based care to Medicare populations
- Collaborate with a multidisciplinary team in a supportive environment
- Utilize advanced resources including onsite labs and specialty consults
Duties & Responsibilities
- Deliver comprehensive primary care to a panel of Medicare patients
- Emphasize preventive care, chronic disease management, and quality outcomes
- Conduct 13-16 patient visits per day, with scheduled time for new and established patients
- Work collaboratively with a care team including scribes, medical assistants, and social workers
- Utilize EMR systems (Canopy and Greenway) for efficient and accurate documentation
- Participate in value-based care initiatives to improve patient outcomes and reduce hospital admissions
- Engage in community health programs and wellness activities organized in the clinic’s community room
- Support patient access, including coordination of transportation within five miles of the clinic
- Contribute to a positive clinic culture focused on patient satisfaction and team collaboration

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
- Board Certification or Board Eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license (non-probationary)
- Strong commitment to value-based, patient-centered care
- Fellowship training in Geriatrics preferred but not required
- Ability to work collaboratively in a team-based clinical setting
- Excellent communication, organizational, and interpersonal skills
- Experience with EMR systems preferred
