Primary Care Physician (MD/DO) - Summary
- Deliver high-quality, value-based primary care services to an assigned panel of senior patients.
- Work collaboratively within a supportive care team that includes a medical assistant, scheduler, and referral specialist.
- Participate in a partnership track with clear advancement opportunities for clinical leaders.
- Enjoy a full-time, weekday schedule in a 100% outpatient care setting.
Duties & Responsibilities
- Provide comprehensive primary care to approximately 18 patients per day, focusing on seniors with multiple chronic conditions.
- Conduct routine health assessments, acute care visits, chronic disease management, preventive screenings, and wellness education.
- Develop and manage individualized care plans, collaborating with in-house specialists such as cardiology and podiatry as needed.
- Utilize value-based care principles to improve patient outcomes and satisfaction.
- Maintain accurate and timely clinical documentation in the electronic medical record (EMR) system.
- Participate in team meetings, quality improvement initiatives, and professional development activities.
- Coordinate with holistic health services available onsite, such as acupuncture, Tai Chi, yoga, and nutrition counseling.
- Ensure effective communication with patients, families, and multidisciplinary care teams.
- Work proactively to achieve patient panel goals and organizational performance metrics.
- Engage in administrative time included in the schedule to support work/life balance.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Residency training in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Commitment to providing patient-centered, value-based care.
- Excellent communication, teamwork, and clinical skills.
- Ability to manage a panel of patients with complex medical needs, particularly seniors with multiple chronic diseases.
- Eligibility for medical licensure in the state as required.
