Our practice focuses on moderate to low-income seniors, many of whom manage multiple chronic diseases. Primary Care Physician (MD/DO) - Summary
- Full-time, outpatient primary care position in Richmond, VA.
- Serve a panel of up to 450 senior patients with 3-5 chronic conditions, focusing on personalized, value-based care.
- Collaborate with a multidisciplinary care team including medical assistants, schedulers, and referral specialists.
- Enjoy a physician-led culture that prioritizes clinical excellence and patient well-being.
Duties & Responsibilities
- Provide comprehensive primary care services to a dedicated panel of senior patients.
- See up to 18 patients per day, ensuring adequate time for quality interactions and follow-up.
- Work Monday through Friday during clinic hours (7:30am – 5:00pm), with last appointments scheduled at 4:15pm.
- Develop individualized care plans, addressing multiple chronic conditions and optimizing patient outcomes.
- Participate in a value-based care model, focusing on delivering measurable improvements in patient health and satisfaction.
- Utilize built-in administrative time to manage documentation and care coordination efficiently.
- Collaborate with in-house specialists (including cardiology and podiatry) and holistic services such as acupuncture, tai chi, yoga, and nutrition counseling.
- Leverage dedicated support team resources for efficient scheduling, referrals, and patient communications.
- Maintain compliance with clinic protocols, quality standards, and regulatory requirements.
- Engage in ongoing professional development and participate in partnership opportunities.
Salary & Benefits
- Competitive base salary starting at $285,000, with additional earning potential through performance bonuses.
- Signing bonus available.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Residency trained in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to value-based care and patient-centered practice.
- Strong interpersonal, communication, and organizational skills.
