Primary Care Physician (MD/DO) – Full-Time, Brooklyn, NY
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative, value-based healthcare network in Brooklyn, NY. This is a full-time opportunity for a board-certified or board-eligible physician in Internal Medicine or Family Medicine. You will work with a supportive care team focused on providing high-quality, equitable healthcare to the local Medicare population. Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive primary care to a diverse Medicare population
- Practice in a supportive, team-based environment with a focus on value-based care

- Utilize modern electronic medical records and in-house clinical resources

Duties & Responsibilities
- Provide direct patient care to Medicare beneficiaries, focusing on prevention, wellness, and chronic disease management
- See an average of 13-16 patients per day, with 20-minute standard visits and 40-minute visits for new or complex cases
- Work Monday through Friday, 8am to 5pm, as part of a collaborative clinical team
- Leverage support from scribes, medical assistants, and social workers to optimize patient care
- Participate in patient education, community room activities, and wellness initiatives
- Utilize on-site laboratories and specialized EMR systems (Canopy, Greenway) to support comprehensive care delivery
- Engage in value-based care initiatives that reward positive patient outcomes and quality metrics
- Maintain accurate, timely documentation and adhere to all regulatory and organizational standards

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted medical license in the relevant state
- No current residency commitments
- Fellowship training in Geriatrics preferred but not required
- Bilingual proficiency in Spanish highly desirable
