We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join an innovative network of neighborhood primary care centers providing value-based healthcare to Medicare patient populations. The Primary Care Physician (MD/DO) role is integral to our mission, allowing you to work with a multidisciplinary care team—including scribes, medical assistants, and social workers—to ensure each patient receives holistic care. You will also have access to on-site laboratory services and advanced digital health resources for streamlined diagnostics and specialist consultations.

Primary Care Physician (MD/DO) - Summary
- Deliver patient-centered, value-based primary care to Medicare populations
- Work within a multidisciplinary team for optimal patient outcomes
- Utilize advanced EMR systems for efficient care delivery
- Focus on preventive medicine and reduction of hospital admissions
- Engage with the local community through center-based activities
Duties & Responsibilities
- Conduct comprehensive patient assessments and provide ongoing primary care
- Manage a daily patient volume of approximately 13-16 individuals
- Allocate 20 minutes for standard visits and 40 minutes for new patients or full diagnostic evaluations
- Collaborate closely with care team members, including scribes, medical assistants, and social workers
- Provide care using value-based health models and evidence-based protocols
- Document patient encounters and manage care using EMR platforms (Canopy, Greenway)
- Coordinate specialist consultations via RubiconMD digital platform
- Participate in community engagement and wellness activities (exercise, yoga, events, and more)
- Support and guide patients with access to transportation and on-site laboratory services
- Contribute to a collaborative clinic environment with 3-5 providers per location

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board certification or board eligibility in Internal Medicine or Family Medicine
- Active, non-probationary, unrestricted state medical license
- No current medical residents
- Fellowship training in Geriatrics preferred, but not required
