Primary Care Physician (MD/DO) – Summary
- Deliver comprehensive, value-based primary care to Medicare populations.
- Work collaboratively in a multidisciplinary team environment.
- Focus on preventive medicine and quality outcomes.
- Practice in a supportive, innovative neighborhood primary care center.
Duties & Responsibilities
- Conduct regular and diagnostic visits (20 to 40 minutes per patient) for 13-16 patients daily.
- Work Monday through Friday during daytime hours, ensuring a healthy work-life balance.
- Utilize electronic medical records (EMR) systems such as Canopy and Greenway.
- Collaborate with a care team including scribes, medical assistants, and social workers.
- Coordinate patient care with access to specialists via RubiconMD.
- Engage patients in community room activities such as exercise, yoga, dance, and wellness events.
- Participate in efforts to minimize hospital admissions and promote preventive care.
- Provide patient education and support for chronic disease management.
- Leverage on-site laboratory and phlebotomy services for timely diagnostics.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, non-probationary state medical license.
- Fellowship training in Geriatrics preferred but not required.
- Strong commitment to value-based care and patient-centered outcomes.
- Ability to work effectively in a collaborative, team-based clinical environment.
- Excellent communication and interpersonal skills.
- Residents are not eligible for this position.
