Primary Care Physician (MD/DO) – Summary
- Deliver patient-focused, value-based care to Medicare and adult populations in a modern neighborhood clinic setting.
- Work collaboratively with a multidisciplinary care team including scribes, medical assistants, and social workers.
- Practice preventive medicine while managing chronic and acute conditions.
- Utilize advanced EMR systems (Canopy and Greenway) and on-site laboratory services.
- Promote community engagement through patient activities and educational events.
Duties & Responsibilities
- Provide comprehensive primary care for a panel of adult patients, focusing on preventive health, chronic disease management, and acute care needs.
- Conduct regular and diagnostic visits (20 minutes for routine, 40 minutes for new or full diagnostic appointments).
- Manage a daily patient volume of 13–16, ensuring thorough and attentive care.
- Leverage care team support and technology to streamline patient care and documentation.
- Engage in value-based care delivery, tracking and meeting quality metrics to drive positive patient outcomes.
- Collaborate with interdisciplinary team members to coordinate care and support patient wellness.
- Participate in community room activities that encourage patient engagement and wellness.
- Utilize on-site laboratory and specialist consults via RubiconMD for comprehensive patient management.
- Ensure compliance with all regulatory and quality standards in medical practice.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license in good standing.
- No current residents; this opportunity is for fully licensed, practicing physicians.
- Fellowship training in Geriatrics is preferred but not required.
