Primary Care Physician (MD/DO) - Summary- Deliver value-based, patient-centric primary care to Medicare populations
- Work in a collaborative setting with a multidisciplinary support team
- Focus on preventive medicine, patient outcomes, and satisfaction
- Utilize advanced EMR systems and onsite clinical resources
- Benefit from a manageable patient volume and extended appointment times
- Provide comprehensive primary care services to adult and geriatric patients
- Conduct regular and diagnostic patient visits (20–40 minutes per appointment)
- Collaborate with scribes, medical assistants, and social workers for coordinated care
- Use EMR systems (Canopy and Greenway) to document and track patient care
- Participate in team-based case reviews and quality initiatives
- Lead and participate in preventive health programs and community activities
- Consult with specialists via digital platforms for complex patient needs
- Ensure compliance with quality metrics and value-based care models

- Board certification (or board eligibility) in Internal Medicine or Family Medicine
- Active, unrestricted state medical license (non-probationary)
- Fellowship training in Geriatrics preferred, but not required
- Strong commitment to value-based care and patient outcomes
- No current residents; experienced physicians preferred
- Passionate about providing high-quality, patient-centered care
- Experienced with Medicare and/or geriatric populations
- Comfortable working in a team-based, value-driven clinical environment
- Effective communicator with patients, families, and care teams
- Adaptable, compassionate, and motivated to make a difference
- Onsite laboratory and phlebotomy services available for efficient diagnostics
- Providers per clinic: typically 3-5, including NPs and Physicians
- Access to transportation for patients within a 5-mile radius of the clinic
- Community programs for patient wellness, exercise, and engagement
