Primary Care Physician (MD/DO) – Summary- Full-time opportunity delivering value-based outpatient primary care
- Focus on comprehensive care for seniors with complex health conditions
- Supportive interdisciplinary team and robust clinical resources

- Provide primary and preventive care for a panel of senior patients, averaging 10-12 patient visits per day
- Emphasize continuity of care and long-term patient relationships
- Utilize a value-based care approach to achieve optimal patient outcomes
- Collaborate closely with care teams, including medical assistants, schedulers, and referral specialists
- Maintain compliance with clinical guidelines, including HEDIS metrics
- Participate in quality improvement initiatives and patient satisfaction programs
- Manage clinical documentation and electronic health records efficiently
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Engage in a rotating call schedule as determined by the number of providers in the market
- Promote community wellness through clinic activities and patient education

- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Current, unrestricted, and active license to practice medicine
- Current DEA registration
- Proficiency in office-based procedures: suturing, joint injections, skin biopsies, and basic wound care
- Fellowship training in geriatrics is a plus but not required
- Strong interpersonal, communication, and organizational skills
