Primary Care Physician (MD/DO) - Summary
- Deliver primary care to a panel of senior patients as a full-time provider
- Practice within a value-based care model focused on improved health outcomes
- Work collaboratively with a dedicated care team including medical assistants, schedulers, and referral specialists
Duties & Responsibilities
- Provide comprehensive primary care services to a small panel of approximately 450 seniors, most with 3-5 chronic conditions
- See 18-20 patients per day in a 100% outpatient setting
- Maintain a Monday through Friday schedule, 7:30am to 5:00pm, with the last patient scheduled at 4:15pm
- Incorporate dedicated administrative time into your workday
- Participate in care coordination and quality improvement initiatives
- Leverage in-house consulting specialists in cardiology, podiatry, and other fields when needed
- Engage patients in holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling
- Ensure exceptional patient experience and outcomes through personalized, respectful, and accessible care
- Collaborate with a multidisciplinary team to optimize care delivery and patient satisfaction

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine
- Commitment to delivering high-quality, value-based care
- Excellent communication, organizational, and teamwork skills
