As a Primary Care Physician (MD/DO), you will play a pivotal role in managing the overall health and wellness of Medicare patients in the Chicago area. Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Deliver comprehensive primary care to a panel of Medicare patients, emphasizing preventive medicine and chronic disease management
- Conduct patient visits (13-16 per day) with appointment durations tailored to patient needs (20 minutes for regular visits, 40 minutes for diagnostics and new patients)
- Coordinate with a team of healthcare professionals including scribes, medical assistants, and social workers to optimize patient outcomes
- Utilize on-site laboratory and phlebotomy services for efficient diagnostic workups
- Access specialists through digital platforms to enhance patient care
- Participate in community engagement activities, including exercise classes, wellness events, and patient education sessions
- Maintain accurate and timely documentation using EMR systems (Canopy and Greenway)
- Support organizational goals for reducing hospital admissions and increasing patient satisfaction
- Engage in regular quality improvement initiatives to advance clinical outcomes

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
- Board certification or board eligibility in Internal Medicine or Family Medicine
- Active, unrestricted, and non-probationary state medical license
- Spanish language proficiency strongly preferred
- Fellowship training in Geriatrics is preferred but not required
- Applicants must not be current residents
