Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive, value-based healthcare to Medicare patients in a supportive, patient-centric environment.
- Work with a multidisciplinary team to facilitate optimal patient outcomes and preventive medicine.

Duties & Responsibilities
- Provide compassionate primary care for a panel of Medicare patients, focusing on preventive care and chronic disease management.
- Conduct thorough patient evaluations, including regular visits (20 minutes) and comprehensive diagnostic or new patient appointments (40 minutes).
- Work collaboratively with an on-site care team comprised of scribes, medical assistants, and social workers.
- Utilize in-clinic resources such as on-site laboratory services and access to specialist consultations through RubiconMD.
- Document patient encounters and care plans using Canopy and Greenway EMR systems.
- Promote patient engagement through community room activities such as exercise, yoga, dance, and wellness events.
- Coordinate care and patient transportation within a five-mile radius of the clinic.
- Participate in value-based health initiatives designed to reduce hospital admissions and improve overall patient satisfaction.
- Maintain compliance with all local, state, and federal regulations, and uphold organization policies and clinical standards.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license in good standing.
- Fluent in Spanish (bilingual proficiency required).
- Preference given to candidates with fellowship training in Geriatrics (not required).
- Open to experienced physicians; residents are not eligible for this position.
