Primary Care Physician (MD/DO) – Summary
- Full-time position in a patient-centered, value-based healthcare clinic.
- Focus on delivering high-quality, equitable care to Medicare patient populations.
- Collaborative team environment with robust clinical and administrative support.
Duties & Responsibilities
- Provide comprehensive primary care services to a diverse Medicare patient panel.
- Conduct regular and diagnostic visits, with 20-minute standard visits and 40-minute full diagnostic/new patient appointments.
- Maintain a patient volume of approximately 13-16 patients per day, ensuring individualized attention and care.
- Utilize value-based care models to minimize unnecessary hospital admissions and prioritize preventive medicine.
- Collaborate closely with a care team including scribes, medical assistants, and social workers.
- Leverage EMR systems (Canopy and Greenway) for efficient patient documentation and workflow.
- Participate in community outreach activities such as exercise programs, wellness events, and educational seminars for patients.
- Engage in ongoing quality improvement initiatives and metrics-based performance reviews.
- Coordinate with specialists and ancillary providers using telehealth platforms as needed to ensure comprehensive care.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary state medical license.
- No current residents; practicing physicians only.
- Fellowship training in Geriatrics preferred but not required.
