Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive, value-based primary care for an assigned panel of senior patients
- Work with a dedicated care team including a medical assistant, scheduler, and referral specialist
- Provide outpatient care with no nights or weekends required
- Maintain manageable panel sizes for enhanced patient focus and outcomes
- Participate in a partnership track with no buy-in, offering long-term professional growth
Duties & Responsibilities
- Conduct thorough patient assessments and develop individualized care plans
- Manage a patient panel with an average daily volume of 18-22 patients, focusing on seniors with 3-5 chronic conditions
- Collaborate with in-house consulting specialists in cardiology, podiatry, and other fields to ensure holistic patient care
- Utilize holistic services such as acupuncture, tai chi, yoga, and nutrition counseling to support patient well-being
- Incorporate administrative time within clinic hours to manage documentation and care coordination efficiently
- Engage in value-based care initiatives to improve health outcomes and patient satisfaction
- Foster a respectful and supportive environment for patients and staff
- Communicate effectively with patients, families, and interdisciplinary team members

Qualifications & Requirements
- Completion of an accredited residency program in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Proficiency in Spanish is required to best serve the patient population
- Demonstrated commitment to value-based, patient-centered care
- Strong communication and teamwork skills
- Experience with senior patient populations and chronic disease management is preferred
