Physicians are empowered to practice medicine in a supportive environment equipped with onsite labs, phlebotomists, and digital access to specialist consultations. With an average patient volume of 13-16 per day and visit durations tailored for comprehensive care, you can focus on building meaningful patient relationships and delivering thorough diagnostic workups.

Primary Care Physician (MD/DO) - Summary- Full-time, value-based care role serving Medicare patients in New Haven, CT
- Focus on high-quality, outcome-driven healthcare delivery
- Multidisciplinary team environment with advanced resources and support
- Deliver comprehensive primary care to Medicare patient populations
- Conduct regular patient visits (20 minutes) and new patient/diagnostic visits (40 minutes)
- Collaborate with a care team including scribes, MAs, and social workers
- Utilize advanced EMR systems (Canopy and Greenway) for documentation and patient management
- Order and interpret labs with onsite phlebotomy support
- Engage in value-based healthcare practices focusing on preventive medicine and reduced hospital admissions
- Participate in patient-centered community activities and initiatives
- Coordinate with specialists via telehealth consultation platforms

- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board certification or board eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license
- Fellowship training in Geriatrics preferred but not required
- No current residents
- Demonstrated commitment to value-based care and preventive medicine
- Excellent communication, collaboration, and patient engagement skills
- Experience working with Medicare populations is a plus
- Comfortable working with multidisciplinary care teams
- Proficient in EMR usage and adaptable to new technology
