Primary Care Physician (MD/DO) – Summary
- Full-time role focused on outpatient primary care for seniors
- Work in a physician-led, mission-driven, value-based care organization

Duties & Responsibilities
- Manage a small patient panel (maximum 450 patients), primarily seniors with multiple chronic conditions
- Conduct comprehensive outpatient primary care visits (average 18 patients per day)
- Develop and maintain personalized care plans to improve patient outcomes
- Collaborate with a dedicated support team, including MA, scheduler, and referral specialist
- Engage in value-based care practices emphasizing prevention, wellness, and chronic disease management
- Participate in clinic hours Monday through Friday, with no night or weekend responsibilities
- Allocate administrative time within the work schedule for documentation and care coordination
- Refer patients to in-house consulting specialists as needed (e.g., cardiology, podiatry)
- Advise on holistic health services available within the clinic (acupuncture, Tai Chi, yoga, nutrition)

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine

- Strong clinical judgment, communication, and teamwork skills
