Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Manage a patient panel of up to 450 seniors, typically with 3-5 chronic conditions.
- Conduct thorough patient assessments and develop individualized care plans.
- Provide ongoing management of chronic diseases and preventive care.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist.
- Utilize value-based care principles to achieve optimal patient outcomes.
- Maintain timely and accurate medical records.
- Participate in interdisciplinary case reviews and quality improvement initiatives.
- Refer patients to in-house consulting specialists when necessary.
- Ensure compliance with all clinical and regulatory standards.
Salary & Benefits
- Competitive base salary starting at $260,000.
- Annual incentive bonus potential of up to $20,000 in the first year, with increased earning potential as a partner.
- Partnership track available, typically within 12-18 months, with no buy-in required.
- Signing bonus up to $50,000.

Qualifications & Requirements
- MD or DO degree from an accredited medical school.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
