Primary Care Physician (MD/DO) - Summary
- Full-time position focused on outpatient primary care

Duties & Responsibilities
- Provide comprehensive primary care for a panel of up to 450 patients
- See approximately 18 patients per day, allowing for in-depth visits and relationship building
- Develop and manage care plans for seniors with 3-5 chronic conditions
- Collaborate with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist
- Participate in care coordination with in-house consulting specialists (e.g., Cardiology, Podiatry) and holistic health professionals
- Utilize administrative time included in your schedule to maintain thorough documentation and care plan management
- Ensure high-quality, value-based care delivery in a 100% outpatient setting
- Work within standard clinic hours (Monday-Friday, 7:30am-5:00pm; last patient at 4:15pm)
- Engage in professional development and partnership opportunities
Salary & Benefits
- Competitive base salary starting at $260,000 with incentive bonuses
- Partnership track with no buy-in, typically achievable within 12-18 months

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine
- Eligibility to obtain and maintain a valid medical license
- Open to candidates with current visa status
