Primary Care Physician (MD/DO) – Summary
- Full-time role delivering comprehensive primary care to adults 65+, with a focus on preventive and chronic disease management.
- Practice medicine in a value-based, patient-focused environment that emphasizes quality care and long-term health outcomes.
- Integrated care team support, including advanced practitioners, medical assistants, social workers, lab, and pharmacy staff.

Duties & Responsibilities
- Provide primary medical care to a panel of adult patients, focusing on individuals aged 65 and older.
- Deliver patient visits averaging 20-40 minutes each, with a daily census of 12-15 patients.
- Coordinate with an interdisciplinary team, including nurse practitioners, physician assistants, social workers, and pharmacy staff, to ensure holistic patient care.
- Lead preventive care initiatives, manage chronic conditions, and facilitate appropriate referrals for specialty services.
- Document patient care accurately within the electronic medical record (EMR) system, utilizing best practices for efficiency and compliance.
- Participate in a comprehensive, three-week onboarding program focused on Value-Based Care principles, documentation standards, and integrated care workflows.
- Engage in ongoing learning and quality improvement efforts to enhance patient outcomes and organizational performance.

Qualifications & Requirements
- MD or DO degree from an accredited institution.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Experience in geriatric care or working with older adult populations preferred.
- Familiarity with HEDIS measures a plus.
- Previous Value-Based Care experience preferred but not required; training provided during onboarding.
- Ability to work collaboratively within a multidisciplinary healthcare team.
- Current and unrestricted medical license to practice.
