Primary Care Physician (MD/DO) – Summary
- Deliver comprehensive outpatient primary care to senior patients with multiple chronic health conditions
- Work within a physician-led, value-based care model focusing on patient relationships and outcomes
- Serve an underserved population, making a tangible difference in community health
Duties & Responsibilities
- Provide direct patient care for a panel of up to 450 seniors, averaging 18-20 patients daily
- Conduct thorough health assessments, develop personalized treatment plans, and manage chronic conditions
- Collaborate with an integrated care team including medical assistants, schedulers, and referral specialists
- Participate in value-based quality initiatives and contribute to continuous improvement of care delivery
- Engage patients through regular visits and build lasting relationships for better health outcomes
- Utilize clinic resources such as in-house consulting specialists and holistic health services to support patient well-being
- Maintain accurate and timely medical documentation in compliance with regulatory standards
- Participate in clinic meetings and interdisciplinary care coordination
- No required nights or weekend shifts; work Monday through Friday, with administrative time built into your schedule

Qualifications & Requirements
- MD or DO degree from an accredited institution
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certification or Board Eligibility in IM, FM, or Geriatric Medicine
- Commitment to value-based, patient-centered healthcare
- Demonstrated ability to work collaboratively within a multidisciplinary team
- Strong clinical, communication, and interpersonal skills
