Primary Care Physician (MD/DO) – Summary- Full-time role focusing on value-based, outpatient care for seniors
- Physician-led, mission-driven organization dedicated to transforming healthcare delivery
- Collaborative, multidisciplinary team environment with dedicated clinical support
- Provide comprehensive primary care services to a panel of up to 450 patients, primarily seniors with multiple chronic conditions
- Conduct in-person consultations, assessments, and follow-up care for 18-22 patients per day
- Develop and implement individualized, value-based treatment plans
- Collaborate with in-house consulting specialists (e.g., cardiology, podiatry) and holistic care providers
- Coordinate care with dedicated support staff, including medical assistants, schedulers, and referral specialists
- Utilize administrative time built into the schedule to complete documentation and care coordination tasks
- Participate in ongoing quality improvement initiatives aimed at enhancing patient outcomes
- Maintain up-to-date clinical documentation in compliance with organizational standards and regulatory requirements
