In this full-time role, you will see a maximum of 18 patients per day, all within a 100% outpatient setting. The patient population consists primarily of seniors with complex medical needs, typically managing three to five chronic diseases. The organization implements a holistic approach to healthcare, offering not only primary care but also in-house specialist consultations and wellness services such as acupuncture, tai chi, yoga, and nutritional counseling. Patients in need of assistance are provided with complimentary door-to-door transportation, ensuring consistent access to medical care.

Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Provide comprehensive primary care services to a panel of up to 450 patients, predominantly seniors with multiple chronic conditions
- See up to 18 patients per day in a 100% outpatient clinic setting
- Conduct routine exams, manage chronic diseases, and develop individualized treatment plans
- Collaborate with an interdisciplinary team including in-house specialists, medical assistants, schedulers, and referral coordinators
- Participate in value-based care initiatives to improve patient outcomes and satisfaction
- Leverage holistic services such as acupuncture, tai chi, yoga, and nutrition counseling to support patient health
- Ensure thorough documentation and compliance with organizational policies and clinical protocols
- Maintain a work schedule of Monday through Friday, with clinic hours from 7:30 AM to 5:00 PM
- Utilize scheduled administrative time for patient care coordination and follow-up
- Engage in ongoing professional development and quality improvement activities

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Completion of residency training in Internal Medicine, Family Medicine, or Geriatric Medicine
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
- Eligible for medical licensure in Missouri
- Strong commitment to value-based care and serving the senior population
- Excellent communication, interpersonal, and organizational skills
- Ability to work collaboratively within a multidisciplinary team
