As a Primary Care Physician (MD/DO) in Beaumont, TX, you will work with an interdisciplinary care team to address the unique needs of older adults, many of whom are Medicare Advantage patients with a focus on chronic disease management and preventive care. You will experience manageable patient census numbers, extended visit lengths, and robust support so you can focus on delivering thorough, compassionate care. Primary Care Physician (MD/DO) - Summary- Provide comprehensive primary care to adults aged 65+, with a focus on chronic disease management and preventive health.
- Work within a supportive, value-based model that allows for deeper patient relationships and better health outcomes.

- Manage a patient panel of older adults, ensuring thorough preventive care and chronic disease management.
- Conduct patient visits (12-15 patients per day) with 20-40 minute appointment times to allow for individualized care.
- Collaborate with an integrated care team, including NP/PA, medical assistants, social workers, pharmacy and lab support, and referral coordinators.
- Document all patient encounters accurately using EMR, optimizing workflows for efficiency and compliance.
- Participate in ongoing training and development, including value-based care and EMR best practices.
- Engage in quality improvement initiatives and contribute to a culture of continuous clinical excellence.

- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Experience with geriatric populations preferred.
