Primary Care Physician (MD/DO) - Summary
- Deliver preventive and chronic disease care to a dedicated senior patient population, primarily adults 65+ with Medicare Advantage coverage.

Duties & Responsibilities
- Manage a patient panel of adults aged 65 and older, focusing on prevention, chronic disease management, and coordination of specialty care as needed.
- Conduct patient visits with ample time, typically 20-40 minutes per appointment, to ensure comprehensive assessments and personalized care plans.
- Collaborate closely with an integrated care team, including Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, laboratory and pharmacy personnel, and referral coordinators.
- Participate in a supportive onboarding program, including a 3-week physician learning journey focused on value-based care principles, documentation best practices, EMR optimization, and integrated care workflows.
- Maintain up-to-date documentation in the electronic medical record (EMR) system, currently Athena, as the organization transitions to new platforms.
- Contribute to a positive, impact-driven work environment by engaging in quality improvement initiatives and supporting organizational goals.

Qualifications & Requirements
- MD or DO degree with Board Certification or Board Eligibility in Internal Medicine or Family Medicine.
- Experience caring for geriatric patients preferred; new graduates or experienced physicians with an interest in senior care are welcome to apply.
- Familiarity or experience with value-based care and HEDIS measures is a plus; training is provided for those new to this model.
- Commitment to team-based, patient-centered care and strong communication skills.
- Eligibility for medical licensure in the state of Florida.
