Primary Care Physician (MD/DO) - Summary- Deliver preventive and chronic disease care to a dedicated senior patient population, primarily adults 65+ with Medicare Advantage coverage.
- Work in a value-based care model focused on building strong patient relationships, improving health outcomes, and providing care in a supportive, team-driven environment.

- Manage a patient panel of adults aged 65 and older, focusing on prevention, chronic disease management, and coordination of specialty care as needed.
- Conduct patient visits with ample time, typically 20-40 minutes per appointment, to ensure comprehensive assessments and personalized care plans.
- Collaborate closely with an integrated care team, including Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, laboratory and pharmacy personnel, and referral coordinators.
- Participate in a supportive onboarding program, including a 3-week physician learning journey focused on value-based care principles, documentation best practices, EMR optimization, and integrated care workflows.
- Maintain up-to-date documentation in the electronic medical record (EMR) system, currently Athena, as the organization transitions to new platforms.
- Contribute to a positive, impact-driven work environment by engaging in quality improvement initiatives and supporting organizational goals.
