As a Primary Care Physician (MD/DO), you will be responsible for providing comprehensive care to seniors, focusing on managing multiple chronic diseases and preventative health. Primary Care Physician (MD/DO) – Summary- Full-time position providing primary care to seniors with complex chronic conditions
- Focus on relationship-building and patient-centered care
- Small patient panel for enhanced engagement and better outcomes
- Supportive, collaborative team environment with dedicated medical assistants and care coordinators
- Located in Deerfield Beach, FL, an area known for its beautiful beaches, vibrant community, and easy access to South Florida amenities
- Deliver comprehensive primary care to a panel of up to 450 patients
- Manage and coordinate care for seniors with multiple chronic diseases
- Conduct patient visits Monday through Friday during clinic hours (7:30am – 5:00pm; last patient scheduled at 4:15pm)
- Average daily patient volume of 18-22
- Utilize value-based care principles to achieve optimal patient outcomes
- Collaborate with an in-house, multidisciplinary team including consulting specialists (Cardiology, Podiatry, etc.)
- Provide holistic care with access to services such as acupuncture, Tai Chi, yoga, and nutrition counseling
- Participate in administrative and care team meetings as scheduled
- Ensure documentation and compliance with all organizational and regulatory standards
- No nights or weekends required; 100% outpatient focus
- Competitive base salary (starting at $250k+)
- Incentive bonus program (up to 10% of base salary)

- Medical degree (MD or DO) from an accredited institution
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Bilingual (Spanish-speaking) strongly preferred
- Commitment to providing compassionate, patient-centered care
- Strong communication, organizational, and teamwork skills
- Experienced in managing seniors with multiple chronic conditions
- Comfortable practicing value-based care in a team-oriented environment
- Dedicated to building long-term relationships with patients
- Skilled in working collaboratively with interdisciplinary teams
- Fluency in Spanish is a significant asset
- Clinic provides access to in-house consulting specialists and holistic health services
- All care is 100% outpatient; no inpatient responsibilities
