Primary Care Physician (MD/DO) - Summary- Provide comprehensive outpatient primary care to a panel of adult and senior patients, with a focus on those managing multiple chronic conditions.
- Work as part of a collaborative, multidisciplinary care team in a physician-led, value-based practice setting.
