Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Conduct thorough patient assessments, develop and implement care plans for chronic and acute conditions.
- Maintain regular follow-up with patients to monitor progress, address needs, and adjust treatment plans as necessary.
- Collaborate with in-house specialists (e.g., cardiology, podiatry) and holistic health providers (e.g., acupuncture, yoga).
- Lead and participate in value-based care initiatives focused on improving patient outcomes and reducing unnecessary healthcare costs.
- Utilize administrative time for charting, care coordination, and interdisciplinary collaboration.
- Supervise and work closely with your dedicated care team, including a medical assistant, scheduler, and referral specialist.
- Educate patients and families about chronic disease management, healthy lifestyle choices, and preventative care.
- Uphold all documentation, coding, and regulatory requirements in accordance with healthcare standards.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Completion of an accredited residency in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Board certification or eligibility in Family Medicine (FM), Internal Medicine (IM), or Geriatrics.
- Commitment to value-based, patient-centered care.
- Experience working with adult and senior populations managing multiple chronic diseases is preferred.
- Bilingual (Spanish) skills preferred but not required.
