Primary Care Physician (MD/DO) – Summary
- Full-time Primary Care Physician role focused on adults aged 65+ in Savannah, GA
- Emphasis on preventive care and chronic disease management for a Medicare Advantage patient population
- Supportive, value-based care model that allows for extended patient visits and meaningful relationships

Duties & Responsibilities
- Provide comprehensive primary care to adults 65+ with a focus on prevention and chronic disease management
- Conduct patient visits averaging 12-15 per day, allotting 20-40 minutes per appointment to ensure thorough care
- Collaborate with an integrated care team that includes Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, and pharmacy/lab staff
- Participate in team meetings and case reviews to ensure optimal patient outcomes
- Utilize electronic medical records (EMR), with training and support provided for system optimization
- Engage in continuous quality improvement initiatives aligned with value-based care goals
- Educate patients on preventive health strategies, medication adherence, and chronic disease self-management
- Coordinate care with specialists and provide referrals as necessary
- Uphold all organizational policies, procedures, and regulatory standards
Salary & Benefits
- Competitive first-year compensation with total earnings potential over $300,000+, including base salary and performance incentives
- Base salary range: $235,000 to $270,000, commensurate with experience
- Commencement bonus up to $75,000 and annual performance bonus (20% of base salary)

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Experience in geriatric medicine preferred; willingness to work with older adult population required
- Experience with value-based care and HEDIS measures is a plus, but not required—training provided
- Strong commitment to preventive care and chronic disease management
- Excellent communication and collaboration skills
