Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive primary care to adults aged 65+, focusing on preventive medicine and chronic disease management.
- Work within a value-based care framework to achieve improved patient outcomes and satisfaction.
- Collaborate closely with a multidisciplinary team, including nurse practitioners, physician assistants, medical assistants, social workers, and pharmacy/lab professionals.
- Practice in a supportive, patient-centered environment in the vibrant community of Columbia, SC.
Duties & Responsibilities
- Provide primary care services to a panel of adult patients, with a focus on those aged 65 and older.
- Conduct patient visits averaging 20-40 minutes each, allowing for thorough assessment and preventive care planning.
- Manage a patient census of 12-15 individuals per day, ensuring each receives high-quality, individualized attention.
- Implement value-based care best practices, including evidence-based documentation and care coordination.
- Participate in a comprehensive onboarding program, including a 3-week training on value-based care principles, EMR optimization, and workflow integration.
- Engage and collaborate with the care team to address the medical, social, and behavioral health needs of patients.
- Ensure compliance with regulatory requirements and quality performance metrics (such as HEDIS).
- Contribute to an impact-driven culture focused on improving senior health outcomes.

Qualifications & Requirements
- MD/DO degree with completion of an accredited residency in Internal Medicine or Family Medicine.
- Active board certification or board eligibility in Internal Medicine or Family Medicine.
- Experience with geriatric populations is preferred.
- Knowledge of or interest in value-based care models is highly valued; training will be provided.
- HEDIS experience is a plus.
