Primary Care Physician (MD/DO) - Summary
- Full-time opportunity for a Primary Care Physician (MD/DO) specializing in Internal Medicine or Family Medicine.
- Serve a predominantly Medicare Advantage population, focusing on adults aged 65+ with chronic disease and preventive care needs.

Duties & Responsibilities
- Provide comprehensive primary care services for adults 65+, emphasizing prevention and chronic disease management.
- Manage a patient census of 12–15 individuals per day, allowing for 20–40 minute visits to foster meaningful patient relationships and thorough care.
- Collaborate with an interdisciplinary care team, including nurse practitioners, physician assistants, medical assistants, social workers, pharmacy, and referral coordinators to deliver integrated care.
- Participate in a 3-week onboarding program focused on value-based care principles, documentation best practices, EMR optimization, and care model workflow.
- Utilize electronic medical records efficiently and adhere to all documentation and quality standards.
- Engage in ongoing learning opportunities and training to stay current with best practices in value-based care and geriatric medicine.
- Contribute to a mission-driven environment, striving to improve health outcomes for older adults in the community.
Salary & Benefits
- Highly competitive base salary, supplemented by commencement and performance bonuses based on experience and achievement.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Board certified or board eligible in Internal Medicine or Family Medicine.
- Geriatric experience preferred but not required.
- Experience in value-based care and HEDIS measures is a plus; training is provided.
- Active and unrestricted medical license to practice as a physician.
- Excellent interpersonal and communication skills, with a team-oriented approach.
