Primary Care Physician (MD/DO) – Summary- Full-time locum tenens physician role in a value-based outpatient setting
- Serving a senior patient population in Lakewood, CO
- Initial assignment of three months, with potential for extension or permanent placement
- All travel, lodging, and malpractice expenses covered
- Supportive integrated care team and advanced EMR system
- Conduct comprehensive outpatient primary care visits for a panel of senior patients
- Maintain longer visit times (12–18 patients per day) to ensure thorough care and strong physician-patient relationships
- Develop and implement individualized care plans focused on prevention, chronic disease management, and patient education
- Collaborate with a multidisciplinary team including nurses, care coordinators, and specialists to optimize patient outcomes
- Utilize a proprietary electronic medical record (EMR) system for documentation, care coordination, and outcome tracking
- Participate in ongoing quality improvement initiatives and support the practice’s value-based care model
- Adhere to all clinical protocols and best practices in line with state and federal regulations
- Engage in regular communication with patients and families, providing education and support as needed
- Competitive hourly rate of $150 per hour

- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
- Board-certified or board-eligible in Internal Medicine or Family Medicine
