Duties & Responsibilities
- Serve as the primary point of contact for patients’ acute, chronic, and preventive healthcare needs
- Develop and implement individualized care plans utilizing evidence-based guidelines
- Collaborate closely with nurse practitioners, medical assistants, care coordinators, and specialists
- Leverage data-driven tools to monitor patient health outcomes and adjust care plans as necessary
- Educate, counsel, and empower patients to manage their health proactively

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Current, unrestricted medical license in California or eligibility to obtain
- Valid DEA certification
- Prior experience in value-based or managed care models preferred
