Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive, preventive, and chronic care management for adult and senior patients
- Work collaboratively in a value-based care model focused on patient outcomes
- Leverage a strong support team including APPs, case managers, and care coordinators
- Practice in a full-time, outpatient setting with no nights or weekends
Duties & Responsibilities
- Conduct thorough medical assessments and provide ongoing primary care to a diverse adult and senior patient population
- Emphasize preventive care, wellness, and chronic disease management in all patient interactions
- Participate in daily high-risk huddles to proactively address the needs of complex patients
- Educate patients and families on health maintenance, preventive strategies, and disease management
- Coordinate appropriate referrals to specialists and facilitate seamless continuity of care
- Maintain accurate, detailed documentation and ensure compliance within the EMR system
- Collaborate effectively with multidisciplinary care teams to deliver high-quality outcomes
- Foster strong patient relationships built on trust, communication, and respect
Salary & Benefits
- Competitive base compensation ranging from $280,000 to $350,000, commensurate with experience
- Annual performance bonus eligibility up to 20%

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Current, unrestricted state medical license and DEA registration
- Experience managing complex or chronically ill patient populations preferred
- Strong communication, collaboration, and patient relationship skills
