Primary Care Physician (MD/DO) - Summary- Full-time Primary Care Physician (MD/DO) role in Far Rockaway, NY
- Deliver comprehensive, value-based healthcare to Medicare patients
- Work within a collaborative, team-oriented clinic environment
- Focus on outcomes, preventive medicine, and patient satisfaction
- Provide primary care services to a panel of Medicare patients
- Collaborate with a care team including a scribe, medical assistant, and social worker
- Utilize onsite resources such as laboratory services and phlebotomy
- Leverage RubiconMD for specialist consultations as needed
- Document patient care using EMR systems (Canopy, Greenway)
- Participate in value-based care initiatives to improve health outcomes
- Engage with patients through community room activities and wellness programs
- Maintain a manageable patient volume (average 13-16 patients per day)
- Conduct regular and diagnostic visits (20-40 minutes per visit)
- Promote preventive care and chronic disease management
- Competitive base salary ($220k-$250k) plus performance-based bonus incentives

- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board certification or board eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license
- Not accepting resident physicians
- Fellowship training in Geriatrics preferred, but not required
