Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Provide comprehensive primary care services to a Medicare patient population, focusing on preventive medicine and chronic disease management.
- Work collaboratively as part of a large care team including scribes, medical assistants, and social workers to deliver patient-centered care.
- Manage patient volumes averaging 13-16 patients per day, with 20-minute regular visits and 40-minute new or diagnostic visits.
- Utilize on-site laboratories and coordinate with specialists via RubiconMD for enhanced patient care.
- Document care using electronic medical records (Canopy and Greenway EMR systems).
- Promote and participate in community room patient activities such as exercise classes, wellness events, and health education sessions.
- Engage in value-based care initiatives aimed at reducing hospital admissions and improving patient outcomes.
- Support patient transportation needs within five miles of the clinic using provided van services.
- Collaborate with an average of 3-5 providers per clinic, including nurse practitioners and fellow primary care physicians.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification, or eligibility, in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license in good standing.
- Fellowship training in Geriatrics preferred but not required.
- Commitment to value-based care and improving patient outcomes.
- No current residents; this opportunity is for fully credentialed physicians.
