Primary Care Physician (MD/DO) – Summary
- Deliver comprehensive primary care to Medicare patient populations in a value-based care setting.
- Work collaboratively with a multidisciplinary care team to optimize patient outcomes.
- Utilize on-site lab services and telehealth consult platforms to enhance diagnostic and treatment capabilities.

Duties & Responsibilities
- Provide direct patient care, focusing on prevention, chronic disease management, and health promotion.
- Maintain a patient volume of approximately 13-16 patients per day, allowing for thorough visits (20 minutes for routine, 40 minutes for new and complex cases).
- Collaborate with care team members, including scribes, medical assistants, and social workers, to deliver coordinated care.
- Participate in community-based wellness activities, such as exercise classes, educational events, and patient engagement programs.
- Document all patient interactions and care plans accurately in the EMR system (Canopy and Greenway).
- Follow value-based care protocols designed to reduce hospital admissions and improve patient health outcomes.
- Leverage specialist access through teleconsultation platforms for comprehensive patient management.
- Engage in quality improvement initiatives and meet established quality metrics for bonus eligibility.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certified or board eligible in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license in good standing.
- Ability to work full-time in a value-based primary care setting.
- No current residents; fellowship training in Geriatrics is preferred but not required.
