Primary Care Physician (MD/DO) – Summary
- Deliver value-based, patient-focused primary care to Medicare populations
- Collaborate with large care teams including scribes, medical assistants, and social workers
- Utilize state-of-the-art clinic resources to ensure optimal patient outcomes
- Enjoy a supportive work environment and innovative care delivery model
Duties & Responsibilities
- Provide comprehensive primary care, including preventive medicine, chronic disease management, and acute care
- Conduct patient visits (13–16 per day) with 20-minute regular appointments and 40-minute new patient or diagnostic visits
- Work collaboratively with interdisciplinary team members to coordinate patient care
- Utilize EMR platforms (Canopy and Greenway) for documentation and care management
- Participate in quality improvement initiatives and contribute to value-based care goals
- Engage patients in community room activities including exercise, wellness events, and educational sessions
- Ensure adherence to all clinical, ethical, and regulatory standards
- Support and mentor colleagues, including nurse practitioners and other primary care team members

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license
- Commitment to providing high-quality, value-based care
- Fellowship training in Geriatrics is preferred but not required
- Residency training completion (no current residents)
