Primary Care Physician (MD/DO) – Summary
- Provide primary care to adult and senior Medicare patients in a value-based, team-oriented clinic setting
- Deliver comprehensive, patient-centered care focused on preventive medicine and wellness
- Collaborate with interdisciplinary care teams, including scribes, medical assistants, and social workers
- Utilize modern EMR systems and access consults with specialists via digital platforms
- Contribute to a positive clinic culture with community-oriented programs and activities
Duties & Responsibilities
- Conduct thorough patient assessments and develop individualized care plans
- Manage a daily patient volume averaging 13-16 patients, allowing for meaningful interaction
- Perform regular and diagnostic visits, with 20-minute slots for established patients and 40 minutes for new or complex cases
- Coordinate care with on-site laboratory and phlebotomy services
- Participate in value-based care initiatives and quality improvement programs
- Engage patients in preventive health measures, chronic disease management, and wellness education
- Document patient encounters accurately using Canopy and Greenway EMR systems
- Support clinic-based activities, including transportation coordination for patients and wellness events
- Collaborate with fellow providers to deliver consistent, high-quality care

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board Certification or Board Eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license in good standing
- No current residency status (not open to residents)
- Fellowship training in Geriatrics preferred but not required
