Primary Care Physician (MD/DO) - Summary- Full-time position in a value-based primary care model
- Focus on Medicare patient population
- Team-based care with support from allied health professionals

- Provide comprehensive primary care services to assigned patients
- Develop personalized care plans emphasizing prevention, chronic disease management, and wellness
- Collaborate with care team members including scribes, medical assistants, and social workers
- Utilize onsite laboratory and access to specialty consultations through telemedicine
- Maintain thorough and accurate documentation using EMR systems (Canopy and Greenway)
- Participate in clinic activities and contribute to community wellness initiatives
- Engage in ongoing quality improvement and value-based care programs

- Graduate of an accredited Medical Doctor (MD) or Doctor of Osteopathy (DO) program
- Board certified or board eligible in Internal Medicine or Family Medicine
- Active, unrestricted, non-probationary state medical license
- No residents; experienced physicians only
- Fellowship training in Geriatrics preferred but not required
- Passionate about value-based, patient-centered care
- Demonstrates strong clinical acumen and interpersonal skills
- Collaborative and comfortable working in a multidisciplinary team
- Committed to professional development and continuous learning
- Experience with Medicare patient populations is a plus
