At our organization, Primary Care Physicians (MD/DO) work within a value-based care model, emphasizing quality over quantity. Our approach is designed to reduce hospital admissions, enhance patient outcomes, and provide a supportive, team-based work environment. You will have the opportunity to serve 13-16 patients per day, allowing for deeper patient relationships and more meaningful care. With access to advanced EMR systems such as Canopy and Greenway, on-site laboratory services, and a dedicated care team—including scribes, medical assistants, and social workers—you will have the tools and support necessary to deliver excellent patient-centered care.
Our Portsmouth, VA location offers a vibrant, welcoming community with a strong focus on health and wellness. Our clinics provide amenities such as a community room for patient activities, including exercise classes, yoga, dance, and wellness events. Patients benefit from convenient transportation services within a five-mile radius, further enhancing access and patient engagement. You will be a part of a team typically consisting of three to five providers per clinic, fostering collegiality and peer support.
As a Primary Care Physician (MD/DO), you will work a consistent Monday through Friday, 8am to 5pm schedule, with 20-minute standard visits and 40-minute slots for full diagnostic and new patient appointments. This structure ensures you have the time to deliver thoughtful, comprehensive care and participate in ongoing professional development. Primary Care Physician (MD/DO) - Summary- Provide value-based primary care for Medicare patients in a team-oriented clinical setting
- Focus on preventive medicine, chronic disease management, and patient wellness

- Deliver comprehensive, patient-centered primary care services, including preventive screenings, chronic disease management, and acute care
- Manage a daily patient volume of 13-16, allowing for longer visit times and personalized care
- Work collaboratively with a care team consisting of scribes, medical assistants, and social workers
- Utilize clinic resources such as on-site labs and access to specialist consultation platforms (e.g., RubiconMD)
- Document patient care efficiently within Canopy and Greenway EMR systems
- Participate in clinic-based activities and patient engagement programs, including exercise and wellness events
- Contribute to a culture of continuous quality improvement and value-based healthcare

- No current residents; seeking independently practicing physicians
