Primary Care Physician (MD/DO) – Charlotte, NC – Full-time
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative value-based healthcare network in Charlotte, NC. Primary Care Physician (MD/DO) – Summary
- Deliver direct, value-based primary care services to a Medicare patient population in Charlotte, NC on a full-time basis
- Work within a supportive, multidisciplinary team to achieve the best possible patient outcomes
- Utilize innovative resources and technology to enhance the patient and provider experience
Duties & Responsibilities
- Provide comprehensive primary care to 13-16 patients per day, focusing on preventive and chronic disease management
- Conduct regular 20-minute visits and 40-minute diagnostic/new patient appointments
- Collaborate with a care team including scribes, medical assistants, and social workers for holistic patient management
- Utilize on-site laboratory services, access to specialist consultations through RubiconMD, and advanced EMR systems (Canopy and Greenway)
- Participate in value-based healthcare delivery, focusing on quality metrics and patient outcomes
- Engage patients in community room activities and wellness programs
- Coordinate with transportation services to ensure patient access to care within a 5-mile radius of the clinic
- Work alongside 3-5 providers per clinic, including nurse practitioners and fellow primary care physicians

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree required
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted, non-probationary medical license
- Fellowship training in Geriatrics preferred but not required
- No residents accepted for this position
