Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Provide comprehensive primary care services to adult and senior patients
- Conduct patient visits, including routine, diagnostic, and new patient evaluations
- Work closely with an integrated care team (including scribes, medical assistants, and social workers)
- Utilize in-clinic labs and access to specialist consultations via RubiconMD
- Document patient care using Canopy and Greenway EMR systems
- Participate in value-based care initiatives to improve patient outcomes and reduce hospital admissions
- Engage in community health activities and patient wellness programs
- Contribute to a positive, team-oriented clinic culture

Qualifications & Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree required
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license in good standing
- Fellowship training in Geriatrics preferred, but not required
- Must not currently be a resident or in residency training

- Average daily patient volume of 13-16, with 20-minute routine visits and 40-minute new/diagnostic appointments

- Team-based practice with 3-5 providers per clinic (mix of NPs and physicians)
