Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive primary care services to Medicare patients in a value-based model
- Work within a multidisciplinary team to provide holistic patient care
- Utilize advanced EMR systems and on-site diagnostic resources
- Focus on preventive care, chronic disease management, and patient education
- Contribute to a collaborative, patient-centered clinical culture
Duties & Responsibilities
- Evaluate, diagnose, and treat adult and geriatric patients with a variety of acute and chronic medical conditions
- Maintain a patient volume of approximately 13-16 patients per day, ensuring quality time with each individual
- Conduct regular visits (20 minutes) and comprehensive evaluations for new/diagnostic patients (40 minutes)
- Work closely with a care team including a scribe, medical assistant, and social worker to coordinate care
- Leverage on-site laboratory services and access to specialists via RubiconMD for enhanced patient management
- Document all patient interactions using EMR systems (Canopy and Greenway)
- Participate in practice-wide initiatives to reduce hospital admissions and drive preventive care
- Engage in community room activities such as group exercise, wellness events, and patient education sessions
- Adhere to all clinical protocols, policies, and regulatory requirements

Qualifications & Requirements
- Graduate of an accredited Medical Doctor (MD) or Doctor of Osteopathy (DO) program
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, non-probationary, unrestricted state medical license
- Fellowship training in Geriatrics preferred, but not required
- Ability to work collaboratively in a team-based environment
- Strong communication, organizational, and clinical skills
- No residents; this opportunity is for fully licensed, practicing physicians
