We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative network of neighborhood primary care centers. Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive, value-based primary care to Medicare patient populations
- Work collaboratively with an interdisciplinary care team
- Utilize advanced EMR systems (Canopy and Greenway) and on-site lab capabilities
- Focus on preventive medicine and chronic disease management
- Be supported by scribes, medical assistants, and social workers for optimal patient care
Duties & Responsibilities
- Conduct patient visits (average 13-16 per day) and provide thorough, patient-centered care
- Perform regular (20-minute) and full diagnostic/new patient (40-minute) visits
- Collaborate with care team members including scribes, medical assistants, and social workers
- Coordinate patient care using EMR systems
- Leverage on-site labs and access to specialist consultations via RubiconMD
- Participate in value-based care initiatives to improve patient outcomes and lower hospital admission rates
- Engage in patient and community activities within the clinic’s community room, including wellness events and educational sessions
- Support transportation initiatives for patients within a five-mile radius of the clinic

Qualifications & Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree
- Board certification or board eligibility in Internal Medicine or Family Medicine
- Active, non-probationary, unrestricted state license
- No current residents (must have completed residency training)
- Fellowship training in Geriatrics preferred but not required
