Primary Care Physician (MD/DO) - Summary
- Full-time clinical role focused on value-based primary care for Medicare patients
- Position based in Brooklyn, NY with a collaborative care team structure
- Emphasis on low patient volume and quality outcomes over quantity of services
- Access to on-site labs, state-of-the-art EMR systems, and multi-disciplinary support
Duties & Responsibilities
- Provide comprehensive primary care to a diverse Medicare population
- Conduct patient visits (13-16 per day) with standard and diagnostic appointment times
- Work with a care team including scribe, medical assistant, and social worker
- Utilize on-site laboratory services and access to specialist consultations
- Document all patient interactions using EMR systems (Canopy and Greenway)
- Participate in preventive medicine, patient education, and value-based care initiatives
- Engage in clinic activities, community events, and patient wellness programs
- Collaborate with other providers within the clinic (NPs and Primary Care Physicians)
- Promote positive patient experiences and outcomes through personalized care plans

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
- Board Certification or Board Eligibility in Internal Medicine or Family Medicine
- Active, unrestricted medical license
- Fellowship training in Geriatrics preferred, but not required
- Bilingual ability in Spanish is an asset
- No current residents; this opportunity is for fully licensed practitioners
