The ideal candidate for the Primary Care Physician (MD/DO) role in Houston, TX will be residency trained in Internal Medicine (IM) or Family Medicine (FM), and board certified or eligible in FM, IM, or Geriatric Medicine. Primary Care Physician (MD/DO) - Summary- Physician-led, mission-driven healthcare organization
- Focus on value-based care for seniors with chronic conditions

- Provide comprehensive primary care to a panel of up to 450 patients, primarily seniors
- Manage an average of 18 patients per day in an outpatient setting
- Develop individualized care plans for patients with 3-5 chronic diseases
- Collaborate with in-house consulting specialists (e.g., cardiology, podiatry)
- Work closely with a dedicated care team including a medical assistant, scheduler, and referral specialist
- Participate in value-based care initiatives to improve patient outcomes
- Utilize administrative time included in the schedule for documentation and care coordination
- Engage in holistic, patient-centered services (e.g., acupuncture, tai chi, yoga, nutrition)
- No night or weekend shifts; maintain a Monday–Friday schedule
- Support organizational mission to honor seniors with affordable, VIP care
- Competitive base salary
- Incentive bonus up to 10% of base salary, paid annually
- Signing bonus up to $50,000
