As a Primary Care Physician (MD/DO), you will manage a small patient panel, allowing for more personalized attention and the ability to deliver truly impactful care. You will work alongside a dedicated support team, including your own medical assistant, scheduler, and referral specialist, ensuring that administrative tasks do not overshadow your clinical focus. We facilitate a collaborative environment with access to in-house consulting specialists in areas such as cardiology and podiatry, as well as holistic services like acupuncture, Tai Chi, yoga, and nutrition counseling, all designed to support whole-person wellness for your patients.

Primary Care Physician (MD/DO) – Summary- Full-time primary care role focused on value-based healthcare for seniors
- Located in Chicago, IL with a collaborative, physician-led team environment
- Emphasis on patient-centered care, quality outcomes, and professional development
- Provide comprehensive outpatient primary care to a panel of up to 450 senior patients
- Diagnose, treat, and manage patients with 3-5 chronic diseases, focusing on holistic health
- Conduct patient visits Monday through Friday during regular clinic hours (7:30am–5:00pm, last patient at 4:15pm)
- Collaborate with in-house consulting specialists and a dedicated support team
- Participate in value-based care initiatives and contribute to quality improvement efforts
- Maintain accurate, timely patient documentation and participate in administrative tasks as scheduled
- Engage in partnership opportunities, with the potential to become a partner within 12–18 months
- Competitive base salary ($250,000–$270,000) with incentive bonus potential up to 10% of base salary
- Signing bonus up to $50,000
- 33 days of paid time off annually

- MD or DO degree from an accredited institution
- Residency trained in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Bilingual in Spanish (required)
