Primary Care Physician (MD/DO) – Summary
- Full-time clinical position focused on outpatient primary care for seniors
- Physician-led, value-based care organization

Duties & Responsibilities
- Provide comprehensive outpatient primary care services to a panel of up to 450 patients, focusing on seniors with multiple chronic conditions
- Conduct patient visits (average 18-22 per day), with ample time allocated for each patient
- Collaborate with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist
- Utilize value-based care principles to drive improved patient outcomes
- Participate in multidisciplinary care planning with in-house consulting specialists (Cardiology, Podiatry, etc.)
- Take advantage of administrative time included in your schedule for documentation and care coordination
- Engage in holistic care delivery, including coordination with providers of acupuncture, Tai Chi, yoga, and nutrition services
- Maintain regular clinic hours, with no nights or weekends required
Salary & Benefits
- Competitive base salary with incentive and signing bonuses

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Bilingual in Spanish (required to meet patient population needs)
- Passion for serving seniors and commitment to value-based, patient-centered care
