Primary Care Physician (MD/DO) - Summary- Full-time, outpatient primary care focused on a senior patient population
- Mission-driven organization dedicated to affordable, high-quality healthcare

- Provide comprehensive primary care services to a panel of senior patients
- Manage a patient panel of up to 450 individuals, averaging 18-22 patients per day
- Deliver care in a 100% outpatient setting with no nights or weekends required
- Practice value-based medicine focused on enhancing patient health outcomes
- Collaborate with a dedicated care team including a Medical Assistant, Scheduler, and Referral Specialist
- Utilize in-house consulting specialists (e.g., Cardiology, Podiatry) and holistic health services
- Participate in administrative and care coordination activities during scheduled time
- Ensure continuity and accessibility of care for patients, including those requiring transportation assistance

- Medical degree (MD or DO) from an accredited institution
- Completion of a residency in Internal Medicine or Family Medicine
- Board certified or board eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine
- Demonstrated commitment to value-based, patient-centered care
- Ability to manage a panel of patients with multiple chronic conditions
- Strong interpersonal, organizational, and communication skills
- Eligibility to practice medicine in Illinois
