Primary Care Physician (MD/DO) - Summary
- Role: Full-time Primary Care Physician (MD/DO), specializing in internal medicine, family medicine, or geriatric medicine.
- Location: Chicago, IL – a vibrant city with diverse communities and a strong healthcare network.
- Setting: 100% outpatient clinic, no nights or weekends required.
- Mission: Deliver accessible, high-quality care to seniors, focusing on chronic disease management and preventive health.
Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 senior patients, averaging 18-22 patients per day.
- Focus on managing 3–5 chronic diseases commonly seen in the senior population.
- Deliver value-based care using evidence-based guidelines and best practices.
- Collaborate with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist, to coordinate patient care and ensure seamless clinical operations.
- Allocate scheduled administrative time to ensure thorough documentation, care coordination, and follow-up.
- Engage in care planning, medication management, preventive screenings, and patient education tailored to the needs of older adults.
- Refer patients to in-house consulting specialists in areas such as cardiology and podiatry as needed.
- Support holistic patient care with access to services like acupuncture, Tai Chi, yoga, and nutrition counseling.
- Participate in quality improvement initiatives and contribute to a culture of continuous learning and development.
Salary & Benefits
- Competitive base salary starting at $250,000, with additional incentive bonuses available.
- Signing bonus opportunities up to $50,000.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Board certification or eligibility in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Completion of an accredited residency in Internal Medicine or Family Medicine.
- Demonstrated commitment to providing compassionate, patient-centered care.
- Strong interpersonal and communication skills, with the ability to work effectively within a multidisciplinary team.
- Experience in value-based care models is preferred, but not required.
